چکیده
Comment 5-FU is an antimetabolite which interferes with normal cell mitosis. It interfers with the S phase and the G2 phase of the cell cycle which correspond, respectively, with the synthesis of DNA and cellular components required for mitosis.' As corneal epithelial cells are constantly undergoing replication they are particularly susceptible to its toxic effects. Reduction of dosage has been suggested as a method of reducing complications including potential toxicity to corneal epithelium.7 However in this case minimal dosage resulted in toxicity to the cornea. Diabetes mellitus is associated with impaired healing of corneal epithelium owing to the increased thickness of the basal lamina resulting in impaired organisation of epithelial cells and anchorage to underlying stromal collagen.910 This case demonstrates that in patients with compromised corneas even judicious use of very small doses of 5-FU may result in serious corneal complications. We feel that band keratopathy is a contraindication to subconjunctival injections of 5-FU and the possible delay in healing of epithelial defects in patients with diabetes means that in such cases it should be used with extreme caution and stopped immediately if epithelial defects appear.

Pseudomonas endophthalmitis associated with scleral necrosis.
Three cases of pseudomonas endophthalmitis associated with scleral necrosis 3 to 10 years after pterygium excision and beta irradiation are described. The value of prophylactic repair of radiation-induced scleral necrosis and early aggressive management of pseudomonas endophthalmitis are emphasized.
متن کاملNecrotising scleritis after bare sclera excision of pterygium.
AIM To report cases of scleral necrosis after simple pterygium excision in which adjunctive treatment was not used. METHODS We reviewed four patients who presented with scleral melt after pterygium excision without the use of adjunctive treatment in the form of beta irradiation, mitomycin C, or thiotepa. Each patient was thoroughly investigated to exclude underlying disease. RESULTS Certain...
متن کاملLate complications of pterygium treatment.
Long-term complications of pterygium excision and beta irradiation in 63 eyes of 57 patients are described. The age of the patients at treatment ranged from 27 to 69 years (mean 48 +/- 11), and complications were assessed 3 to 20 years later (mean 12 +/- 3). The pterygia were excised, leaving bare sclera, and beta irradiation of total dose 750 to 5200 rads (mean 3475 +/- 916) was given, except ...
متن کاملTailored treatment for the management of scleral necrosis following pterygium excision
The present study aimed to investigate the efficacy of tailored treatment for the management of scleral necrosis following pterygium surgery. A series of nine cases of scleral necrosis following pterygium excision between September 2009 and September 2012 were included. In cases where ischemia was the cause of scleral necrosis, Tenon's membrane covering (TMC) surgery was performed. For cases wi...
متن کاملScleral Cross-linking Using Riboflavin and Ultraviolet-A Radiation for Prevention of Axial Myopia in a Rabbit Model.
Myopic individuals, especially those with severe myopia, are at higher-than-normal risk of cataract, glaucoma, retinal detachment and chorioretinal abnormalities. In addition, pathological myopia is a common irreversible cause of visual impairment and blindness. Our study demonstrates the effect of scleral crosslinking using riboflavin and ultraviolet-A radiation on the development of axial myo...
